Circle
of Health is available to all Band members no matter
where you live. Enrollment in the Circle of Health costs
you nothing. We need to know where you live and
information on what other health care coverage is
available to you before we can provide our supplemental
insurance.
If you work for the Mille Lacs Band or a Mille Lacs Band
enterprise you will be automatically be covered.
If you are employed by someone other than the Mille Lacs
Band, the Circle of Health needs to know who your
insurer is so we can make arrangements to make direct
payments to that company for premiums. We will need a
copy of your policy to coordinate the benefits that are
not covered.
If you are receiving Medical Assistance, General
Assistance Medical Coverage, or MinnesotaCare, or a
similar program in your state, you will need to inform
the Circle of Health so we can coordinate with the
state, county, and medical providers to assure payments
on your behalf.
If you live near the reservation, in the Twin Cities or
Duluth and you have no medical coverage, call the Circle
of Health. We will have a customer service
representative to help you determine your needs.
If you live out of Minnesota and are receiving Medical
Assistance you must contact the Circle of Health so we
can make arrangements for any payments on your behalf.
If you live outside on Minnesota and have no insurance
call the Circle of Health at the numbers listed below.
If you are receiving Medicare benefits, The Circle of
Health will purchase supplemental coverage for you.
